Abstract The authors describe a system for generating discharge summaries based on a preprinted form and a database based on the MICRO software package from the Institute of Labor. Forms are initiated and partially filled out by clerical staff, completed by physicians, and entered into the computer by clerical staff. Printed discharge letters are mailed to the attending physician and also included in the permanent medical record. The average time for printing the letter (after data entry is complete) is 3 minutes and the average cost per letter is $1.60.
